How it works

We deploy a comprehensive customer referral, typically for hospital OTs, Social Services officers and so on.
In addition to customisable referral formats the Care and Health Referrers module is able to capture data, securely share with approved networked users. monitor and report transactional activities.
For example, a customer could be discharged from hospital care but require interim or longer term support at home. The discharge would be referred, via our cloud-based platform, employing an ‘intelligent’ form interface (using conditional logic).
An element of the form can include a Referred-To care provider – such as domiciliary carer, social services or telecare service. This nominated service or services could be alerted by an email, SMS etc. Alternatively the form might be digitally forwarded to our own experienced customer engagement team to facilitate the process – including via telephone where needed.
The form could generate a customer record that can be imported into another system, either via standard API or frequent batch upload via common CSV files / Excel etc.
